Authors Stuart G The Bosniak classification for renal cysts was developed in the late 1980s in an attempt to standardize the description and man agement of complex cystic renal lesions. Alterations were made to such a classification in the 1990s and, the last one, in 2005.

When facing such a lesion, the question which needs to be answered is whether this lesion is surgical or not. A study has suggested that a homogeneous mass measuring 70 HU or greater at unenhanced CT has a greater than 99.9% chance of representing a high-attenuation renal cyst. This is classified as a Bosniak type 2 cyst. Related Radiopaedia articles. Although it is widely used and still effective, the classification does not formally incorporate masses identified at MRI or US or masses that are incompletely characterized but are highly likely to be benign, and it is affected by interreader variability and variable reported malignancy rates.

Jonathan McFarlane. The Bosniak renal cyst classification has, thus far, passed the test of time and has been a useful method for diagnosing and suggesting the management of complicated cysts and cystic neoplasms. In the past 25 years, there have been continuous advances in the diagnosis of disease throughout the body owing to the introduction of new technology and the experience gained with its use. In 1986 Bosniak proposed a simple, four category classification system for renal cysts, which has since been widely adopted and has been proven to be sound and reliable[1,2].

Bosniak IIF masses have been followed, and Bosniak III and IV masses historically have been treated unless sub-stantial comorbidities or limited life expectancy would warrant observation instead (10–12). Rationale for Updating the Bosniak Classification The Bosniak classification has withstood the test of time

simple cyst (Bosniak I) well-marginated; thin or imperceptible wall; water attenuation (<20 HU on noncontrast series) non-enhancing <10 HU increase from non-contrast to postcontrast series is non-enhancing; 10-20 HU increase is indeterminate >20 HU increase is enhancement Using Bosniak classification version 2019, cystic renal masses were classified into 12 category I, 19 category II, 13 category IIF, four category III, and 20 category IV by CT and eight category I, 15 category II, 23 category IIF, nine category III, and 13 category IV by MRI.
